Health Screening Questionnaire

We are screening all participants/staff/partners/guests on a daily basis to protect the health and safety of everyone who enters or attends a GROW Program training facility.

Please complete the following information within 12 hours of attending each day of on-site class, prior to boarding the bus, or entering the training facility. If you answered “YES” to questions #1 to #6 or currently experiencing symptoms as described in question #6, please do not proceed to the GROW Program Training Facility.
